Personal Injury Mediation?

During a mediation session is it normal to have the expert witness doctor present? The defendant side has one evaluation doctor and the plaintiff has 4 medical doctors. Does the plantiff has to be present for the mediation or is it just the two legal counsels from both sides? Does the defendant bear the cost of the mediation?
